PHILIPPINE Health Insurance Corporation (PhilHealth) continuously reminds its members that the request for ID cards and Member Data Record (MDR) are free from any charge.
We reiterate that we do not charge any fees for the printing and issuance of PhilHealth ID cards and MDRs. Those who need to have their ID cards and MDRs printed are advised to proceed to the nearest PhilHealth Regional Office, Local Health Insurance Office (LHIO), or PhilHealth Express in your localities and transact with our frontline personnel directly said Ruben A. Basa, the Executive Vice-President and Chief Operating Officer in an advisory released by the Corporation.
Members are cautioned against individuals who offer to assist members in processing and securing their personal data in PhilHealth with payment ranging from P200 to P500 as this act is strictly prohibited under the Anti-Red Tape Act of 2007.
For members requesting for their ID cards and MDRs, a copy of any valid ID shall be presented while authorized representatives shall bring an authorization letter and valid IDs both of the member and the requesting party.
